Cancellation of my broadband service

My account no is alexbo2958 with address 80A Barker Street in Kingsford, I received notice yesterday that you still deduct money from my account, but we have moved out on 14/09/2020 from the house, and  before last month I have applied disconnection of internet, but I have no idea why it is failure, can you help me to disconnect the internet and withdraw my money for the next month. I will attach screen shot from agent for the confirmation of vacation.
