Account cancellation

I have tried calling you on several occasions but I cannot get through. I have tried the Contact Us form but this gives an error on both MS Edge and Google Chrome (ajax.$form.ajaxSubmit is not a function).

I am moving out of my apartment this week and moving in with people that already have internet. I am out of contract. Please cancel my ADSL and home phone bundle as soon as possible.
