Change of ownership - FTTB

 Hi all!
I am organising a change of ownership and I found the online Change of Ownership Form ( https://www.tpg.com.au/forms/Change_of_Ownership_Form.pdf?ver=072716 )

I have completed it, but I am not sure it is the correct form.
I have FTTB, but the form contains references only to NBN and ADSL.
Is it the right form nonetheless?

Hi Gabriele,

Welcome to the Community! 

You can use the change of ownership form for NBN/ADSL. Just put the note "TPG FTTB" on the blank portions of every page.
