How can I cancel my ADSL2 + Home Phone service

I want to cancel my ADSL2 + Home Phone service. My account is "***93". I already talked with cancellation specialist for cancelling the service through phone call. The specialist said she will send an email with cancellation form to my email. But she did not send to me. So please give the email address so that I can send any information about cancelling my account. Thank you.
