Change account name to business

HI, 

my business NBN account is registered under an indiviaul and i am trying to remove this persons name from the account and set the account on the business name , but i have had no luck . 

I have filled out the change of name form sent to me too , but nothinh has happened . 

Please help / guide me to the right procedure etc for this to happen ?
