I'd like to request for refund & cancel ADSL internet

Since your company couldn't resolve my issue of non-working slow internet speed & multiple internet disconnections which i'm encountering over the past month. This has caused significant disruptions.

During the past month, my internet has become very slow & too many times I had no connections. I had contacted you multiple times but your attemps have failed to resolve the issue. I had sent an email few days ago but still haven't received any response.

I'd like to request for the refund for your service disruption over the past month and I would like to cancel your ADSL internet.
