New SIM saying network locked

I just activated my new SIM and my phone is saying it's invalid and that a network locked SIM has been inserted. It's asking me for a PUK code and when I requested one from my account it said incorrect mobile number.
I have tried restarting my phone and playing with the network settings but nothing is working.
Have I been sent a faulty SIM?
