ADSL2+ Cancellation - How to ?

I would like to cancel my existing ADSL+ service that I have. I am unable to find any details around how to do it, except for responses to questions from this community in the form of personal DMs.

Could you please let me know how to do it ? Or give me the email address to which I should send my cancellation request. I am unable to even find an email contact from your contacts page.
