Refund for no service

Hi,

I am looking to get a refund as there was no installation that ended up taking place for my nbn order. Also the wrong address was sent to nbn for installation. And the an additional 20 dollars was recently debited from my account despite cancelling. What are the next steps for getting a refund? I am unhappy with the mistakes and frustrating process to get through my connection journey which ultimately led me to cancel my order.
