Transfer of existing service that is soon to be terminated

Hi,

An existing internet NBN service is soon to be terminated as my residence. I wanted to know if it was possible to transfer the existing service upon termination to myself. I know that there's an ownership transfer form but the holder of the existing service does not have their account username. Would it be possible to have the service immediately set up to myself upon termination?
