Constant Drop outs + Slow Internet

Over the past month, our internet connection has been constantly dropping out or very slow and not close to reaching the speeds of our plan. 

 

Each time it drops out, we have to switch/disconnect the modem, re-plug and it begins working after a while, but then would drop out again soon after. As we need internet for work purposes, this is very frustrating - can I please have someone look into this?

We'd like to check on what's causing the connection dropouts and see what we can do to resolve it.
