VoIP Phone Setup using TPG modem

Hi TPG, 

Issue:

Cannot configure Grandstream GRP2612 VoIP phone.

Facts: 

• NBN FTTP connection.
• Using the TPG supplied modem/router (Netcomm NF12) as per posts listed below
• Have tried multiple settings from online sources, but will not work (including VoIP settings here: https://www.tpg.com.au/support/generalsetting.php)

Help:

Could you please provide the relevant details to configure the phone?
SIP Server

Outbound Server

SIP User ID

Authenticate ID

Authenticate Password

Name
