Voip not working

I have had voip for at least 6 years.  It has been unreliable, often dropping out and requiring a modem reboot, and of fairly poor quality.  Now it doesn't work at all, despite several modem reboots.  The usual lights are all on and I get a dial tone but when I ring a number the handset gives a ringing tone for two rings and then gives an engaged signal.  When I ring my own mobile the voip handset gives the sound of two rings (before cutting out) but the mobile does not register any rings.
