Replacement Modem

Hi. I've been having an issue with my modem today. I am now getting these lights, from the left (orange, green, orange, blank, orange then the rest green.)
The modem is a Huawei HG658.
It will work for roughly 5 minutes but then it goes back to not working.
Any help would be greatly appreciated.

What troubleshooting have you tried so far?

Also, we'd like to perform a line test to confirm that there's no fault causing the connection to drop.
