Asked to enter mobile number & passphrase when dial 121

I'm being asked to enter mobile number & passphrase when dial 121. Every time. It never used to do this & I could just dial 121 and get directly into voicemail, without the additional steps. What's going on?

From today I can't access my voicemail at all. Whenn I dial 121 it just says cannot be connected. I can see I have a few missed voicemails. What do I do now?
