NBN internet and home phone not working

NBN internet and home phone both don't work in the Wishart (QLD) area. This has been happening since 21st of December specifically during the day time after 2pm till late at night. Can someone please investigate this? It's been 2 weeks now and we don't have internet or phone connection at the moment.

I was able to locate your account using your community details and ran initial test. Result shows a possible line fault causing the service not to work.
