Return 4g Modem post 5G upgrade

Hi Team,

 

I have upgraded my 4G service to 5G and was advised that I will receive a return lable and instructions over email. Its been 2 weeks 4 calls to customer care and nothing received till date. Please suggest how do I return the modem and complete the upgrade.
