resign for new plan period- need a new modem

Hi,

I want to resign for an upgraded NBN plan now that my original plan is out of contract, but I need a new modem. Will this new plan come with a new modem, and how can I arrange this ASAP, as our current modem is not good and I would like to get back on NBN anyway now that it is available at my address.
