Where to return faulty modem

Hi I have been asked to return my faulty modem within 14 days, however I have not been provided with a return address. I am currently waiting for my new modem. Will this information come with the new modem?

You may return the faulty modem directly at this address 65 Waterloo Road North Ryde, NSW 2113. Once you shipped the equipment we recommend to notify us with the tracking number together with your account details for documentation purposes.
