Cancel ADSL

Hi,
I called today and after 30 minutes finally got to speak to someone.
I advised I wanted to cancel my service. I was told I could not do this over the phone and would be emailed cancellation forms within a few minutes. It has been 6 hours and I have no received these forms. I want this service cancelled immediately, I will now not be paying for any further services.
Please confirm within 24 hours or I will go to the telecommunications ombudsman to resolve this.
