I can see there are lots of correctable errors coming from your modem, which can cause transfer rate issues (issues with streaming and slow on opening sides).
If u have a spare modem, please try. Also if you are not using homophone,
make sure no filters between modem and socket.
I made some changes on to your line. Hopefully, it will stop connection lost

My NBN service starts from this week getting very slow and disconnects every day, it back to normal when I restart the modem, but after few hours it back to slow speed. All light in the modem is working (light is On), just no speed or very slow, I am used NBN 100 plan
I am experiencing constant drop outs.
It was patchy from day 1 (Feb install) and it continues to be patchy.
I need a reliable internet connections to WFH and School kids from Home too!!!
I am on NBN50 and it is just slow.
